When you make a payment through our services to a physical therapist (as described below in and subject to other provisions of the Agreement), your payment card information is processed through payment processing by a Paypal company. Paypal collects your voluntarily provided payment card information to process your payments in connection with your use of the Services. The use and storage of the information you collect on Paypal is governed by Paypal’s terms of service and privacy policy. The information we store may include your payment card type and the last four digits of the payment card. We may provide you with the option to delete your stored payment card information through your account settings page.


      2.2 Traffic data

We may also automatically collect certain data when you use the Services, such as (1) IP address; (2) domain server; (3) type of device (s) used to access the Services; (4) web browser (s) used to access the Services; (5) refer to the website or other source through which you accessed the Services; (6) geolocation information; And (7) other statistics and information associated with the interaction between your browser or device and the Services (collectively “Traffic Data”).      2.3 HIPAA and PHI

Under a federal law called the Health Insurance Portability and Accountability Act (“HIPAA”), some of the demographic, health and / or health information that Kaenz® collects as part of the benefit of the Services may be considered “protected health information” “PHI” (protected health information) ”. While your decision to electronically sign the Authorization is entirely voluntary, if you do not electronically sign the Authorization, you are not authorized to use the Services. If you choose to electronically sign the Authorization, you agree that Kaibaz may use and disclose your PHI in the same way that it uses and discloses your Personal Information that is not PHI. These uses and disclosures are described in this Privacy Policy. To the extent that any provision in the Authorization is inconsistent with this Privacy Policy or other provisions of the agreement, then the provision in the Authorization only controls with respect to your PHI.


  1. How we collect information

We collect information (including personal information and traffic data) when you use and interact with the services and in some cases, with third-party sources. Such information includes:

  • When you use the interactive tools and services of the Services, such as finding Physical Therapists, searching for available appointments with Physical Therapists, and completing medical history forms (“Case History Forms”) prior to Physical Therapist appointments;

  • When you voluntarily provide information to your Physical Therapist, your Physical Therapist may enter that information into the Services;

  • We may share your Personal Information with your Physical Therapists so that they can refer you and make appointments with other Physical Therapists on your behalf or to conduct analysis on possible health problems or treatments, as long as you choose to use the applicable Services.
  • We may share your Personal Information with Physical Therapists in the event of an emergency.
    We may share your personal information as part of a set of aggregated and anonymous data, in which case the personal information will be de-identified.
